  • Business License
  • Professional or Occupational License
  • Phone Bill of Number Used on Application
You will also need a copy of at least one of the following:
  • Liability Insurance/Bond
  • Business Card or Letterhead Signed by Owner
  • Incorporation Filing
  • Fictitious Name or DBA Filing
  • Voided Business Check
